Wednesday, June 17, 2026 An updated version of the Main & Optional Consent Form Template (Dated 2026Apr29 Posted) was recently posted to align with GCP(R3), to clarify withdrawal language pertaining to mandatory samples, and to address minor administrative updates. The clean version of the template is posted under the Generic Sample Informed Consents tab on the Generic Protocol page: https://www.ctg.queensu.ca/private/generic-protocol. As trial consent forms are modified for open trials, GCP-related changes will be made. This updated version of the template will be used for newly activating trials.
