De-Identifying Supporting Documents

Central Office staff have noticed that, on occasion, supporting documents (radiology, pathology reports) arrive with patient identifiers present (i.e. patient names are not blacked out).

Please ensure that patient identifiers are blacked out locally, and that the Canadian Cancer Trials Group patient ID number is added prior to submission to the Canadian Cancer Trials Group Central Office. As a patient's name may be referenced a number of times in one report, please remember to review the entire document for identifiers.

With respect to informed consent documents, please note that a portion of the patient's signature (i.e. initials or coded identifier) must be visible. As with all aspects relating to patient identifiers and privacy, please ensure local policy is followed as well.
